Make the Service Plan Fields Optional

FillQuick currently assumes that every account will have a monitoring service. And that every customer will be paying a monthly charge. But this is not always the case. Some FillQuick users would like to enter customers that bought the equipment and installation service, but did not buy any monthly service plan. A new field to specify whether the customer has a service plan will be useful. When there is no service plan, the related fields like the "Monitoring company" and "Service package" should be hidden.
